FORT MYERS — Lee Health, one of the largest community nonprofit medical facilities in the region, named John Chomeau chief population health officer.
Chomeau's chief role with the health system will be to improve “population outcomes across the continuum of care,” according to a release. Chomeau will shape Lee Health's approach to value-based care, which focuses on keeping people healthy, as well as improving the health of people with chronic conditions through evidence-based, cost-effective means, the release adds.
Chomeau will also oversee the Physician Hospital Organization and Clinical Integration Network within Lee Health.
“We are excited to have John Chomeau join our team,” Lee Health Chief Medical and Clinical Integration officer Dr. Scott Nygaard says in the statement. “His rich and diverse work experiences make him well-qualified for this new role.”
Chomeau was previously an executive with Magellan Health and was also president and chief operating officer of the Medicare unit at Aetna. “I am very excited to be part of a great organization like Lee Health that is fully committed to improving the health of our community by better aligning our population health strategy,” Chomeau says in the statement.
